1、開辦第二、三類醫療器體經營企業，應向經營所在地食品藥品監管分局申請并提交以下資料
(1)、《北京市醫療器材經營企業許可證申請表》;
(2)、《醫療器材經營企業許可證申請材料登記表》;
(3)、工商行改管理部門出具的企業名稱預技準證明文件成《營業執照》氣印件(校驗口件);
(4)、擬辦企業質量管理負責人的身份證，學歷我者職你證明復印件及個人簡歷
(5)、擬辦企業委量管理人員的身份證、學歷成者職稱證明氣印件
(6)、擬辦企業組織機構與職能或專職質量管理人員的職
(7)，扣辦企業注冊地，倉庫的地理位于要，平自圖(注明商積)，房厚產權證明或者租賃協議(付租優房厚產權證明，下同)復件
(8)、擬辦企業產品圍量管理制案文件及備存合作、設備目錄，奮要二分一應當按照《關療最展經三企業許可加管理辦法》第十二條第二項的規定作出處理決定，
